Basic Function:
The Home Care Assistant provides compassionate, personalized, and supportive care to individuals in their homes. The role focuses on assisting patients with daily living activities, maintaining their comfort, safety, and well-being, and delivering care based on each patient’s individual needs. The position promotes patient independence while working collaboratively with families and healthcare professionals to ensure effective and high-quality care delivery.

Principal Duties and Responsibilities:
1. Personal Care and General Hygiene Assistance:
Assist patients with bathing, dressing, changing clothes, and maintaining personal hygiene.
Support skin care practices and help prevent pressure ulcers through proper hygiene and positioning.
Clean and disinfect patient care equipment and tools according to infection prevention standards.
Maintain cleanliness and organization of the patient’s bed area and surrounding environment.

2. Mobility and Movement Support:
Assist patients with safe movement and transfers within the home, including sitting, standing, and walking.
Provide support with the use of wheelchairs, walkers, and other mobility aids when required.
Ensure patient safety during movement and mobility activities.

3. Food and Hydration Support:
Prepare light meals or serve prepared food according to the patient’s needs.
Assist patients with eating and drinking when they are unable to perform these activities independently.
Monitor and encourage adequate hydration and nutrition as required.

4. Basic Patient Monitoring and Reporting:
Monitor basic vital signs (such as temperature, blood pressure, and glucose levels) when trained and authorized to do so.
Observe and report any changes in the patient’s health condition, behavior, or overall well-being to the healthcare team.
Maintain accurate documentation of daily care activities and services provided.
Immediately report any emergency situations or significant changes in the patient’s condition.

5. Psychological and Social Support:
Provide emotional support and reassurance to patients to enhance comfort and confidence.
Engage patients through conversation, active listening, and meaningful interaction.
Promote a positive and respectful environment that supports the patient’s emotional well-being.

6. Newborn and Maternal Care Support:
Assist with newborn care activities as required.
Support mothers during postpartum recovery and provide assistance with daily maternal care needs.
Promote comfort, safety, and well-being for both mother and newborn.

7. Confidentiality and Professional Conduct:
Maintain strict confidentiality of patient information and respect patient privacy at all times.
Respect and uphold patients’ rights, dignity, and responsibilities.
Comply with company policies, procedures, applicable laws, regulations, and requirements of relevant regulatory authorities.
Demonstrate professional behavior, ethical conduct, and respectful communication in all interactions.
